Health Service School Dental Service
The School Dental Van is visiting Meridan State College and offering FREE dental care.
- Year 9 and 10 students are now being offered appointments.
- Parent/ Guardians need to complete a consent form (attached).
- Please print, complete both sides, sign and return to Student Services before Friday Feb 10th.
- For those that require paper copy- Forms are available at Student Services.
Once forms are returned, dental staff will contact you via SMS to organise an appointment for your student. Students will be called down from class for their appointment. Parents are always welcome to attend.
Appointments will be at the dental van on site at
Meridan State College. (located on driveway next to oval, primary side)
Oral health staff are observing COVID19 safe protocols. If you or your child feels unwell, we ask that you phone dental staff prior to the appointment.
If you have any enquiries, please phone the dental van directly on 0412369343. If staff are unable to answer, please leave a message and they will return your call as soon as possible.
